World Suicide Prevention Day launched by The International Association for Suicide Prevention (IASP) is particularly alive in Italy (www.prevenireilsuicidio.it). One important achievement was the historical meeting between Maurizio Pompili, M.D., Ph.D. and Pope Benedict XVI on the occasion of World Suicide Prevention Day  (WSPD). Here is Maurizio Pompili's description "During those moments, I just told him that I was involved in suicide prevention, that I was supporting IASP, WSPD and I asked him to help us. The Pope very charmingly held tightly both my hands for a few seconds, listened to a few words and then blessed the prevention of suicide. I saw this meeting as a symbolic event that put in the Pope's hands the prevention of suicide after so many centuries of stigmatization towards this phenomenon".

Raceforlife è un evento ideato e realizzato dal Servizio per la Prevenzione del Suicidio.

Partecipare alla Race for Life è un’esperienza unica.

L’obiettivo è  quello realizzare una campagna di sensibilizzazione relativa alla delicata e spesso sottostimata tematica del suicidio e della sua prevenzione presso l’opinione pubblica. Utilizzando il linguaggio dello sport che promuove la solidarietà, la comprensione e la determinazione, l’evento mira a far conoscere un tema spesso stigmatizzato e a promuovere il riconoscimento dei soggetti in crisi. Se approcciati nel giusto modo i soggetti in crisi accettano l’aiuto e testimoniano la loro voglio di vivere.

 

L’Organizzazione Mondiale della Sanità (OMS) considera il suicidio come un problema complesso non ascrivibile ad una sola causa o ad un motivo preciso. Sembra piuttosto derivare da una complessa interazione di fattori biologici, genetici, psicologici, sociali, culturali ed ambientali. Il suicidio, nell’ambito della salute pubblica, è un grave problema che potrebbe essere in gran parte prevenuto; costituisce la causa di circa un milione di morti ogni anno con costi stimabili in milioni di euro secondo quanto indicato dall’Organizzazione Mondiale della Sanitá. Stando ai dati attuali e all’analisi dei tassi di morte per suicidio nel mondo, le stime suggeriscono che nel 2020, le vittime potrebbero salire ad un milione e mezzo. Nel 2000 circa un milione di individui ha perso la vita a causa del suicidio, mentre un numero di individui variabile da 10 a 20 volte più grande ha tentato il suicidio. Ciò  rappresenta in media una morte per suicidio ogni 40 secondi ed un tentativo di suicidio ogni 3 secondi. Questo ci porta a concludere che muoiono più persone a causa del suicidio che per i conflitti armati di tutto il mondo e gli incidenti automobilistici. In tutte le nazioni, il suicidio è attualmente tra le prime tre cause di morte nella fascia di età 15-34 anni; nel recente passato il fenomeno predominava tra gli anziani, ma ai nostri giorni è più allarmante tra i giovani sia in termini assoluti che relativi in un terzo delle nazioni. Questo dato rappresenta una perdita di vite umane più alta rispetto alla perdita di vite causata da guerre ed omicidi ogni anno; tre volte la perdita di vite umane registra nella tragedia dello Tsunami nel sud-est dell’Asia nel dicembre del 2004; inoltre ogni giorno muoiono a causa del suicidio l’equivalente delle vittime causate dall’attacco alle torri gemelle di New York l’11 settembre del 2001.

E’ dunque un fenomeno che non può essere ignorato e vi è la necessità di infrangere quel silenzio e quel composto, ma deleterio, sviamento dal tema attuato quotidianamente.

 Secondo quanto affermato da un documento dell’OMS il suicidio è un problema di grande entità nei paesi europei, soprattutto tra i nuovi stati indipendenti dell’Europa dell’est. I governi di tutte le nazioni del mondo sono impegnati nella lotta contro il suicidio promuovendo la consapevolezza e migliorando gli interventi e le tecniche preventive.

Ogni individuo può fare qualcosa per aiutare a ridurre il numero delle persone che considerano il suicidio come soluzione al loro dolore mentale.

Lo storico incontro tra il padre
della suicidologia Edwin Shneidman, Ph.D. e Maurizio Pompili, M.D. insignito con
lo Shneidman Award 2008 dall'American Association of Suicidology con la
motivazione "outstanding early career contributions to Suicidology". Nel corso
di una lunga collaborazione, Shneidman ha invitato il Dr. Pompili a Los Angeles
per discutere il futuro della suicidologia.

 

Edwin S. Shneidman 1918 - 2009

Edwin S. Shneidman has died at the age of 91 on May 15, 2009 in his home in West Los Angeles. He is commonly considered the father of suicidology as he pioneered the discipline in the fifties starting from a serendipitously study of suicide notes. He was at that time, with Norman Farberow and Robert Litman, the Co-founder of the Los Angeles Suicide Prevention Center, later on the Founder of the American Association of Suicidology and the Founder-Editor of Suicide and Life-Threatening Behavior. He was also Professor of Thanatology Emeritus at UCLA. He coined various words of our field such as psychological autopsy, postvention and psychache to name just a few. Despite his age he never lost the spirit of a researcher and a thinker. He was always prolyphic with ideas, suggestions and in fueling the development of suicidology. He authored and edited some twenty books and his latest one (A commonsense book of death) was published just a few months ago. I spoke with him two days before his death, on his birthday, and he confessed that it was the end but actually found the words to thank me and say goodbye. Despite he was “Waiting for death” as he stated, he conserved his scientific interested in the end of life process, indefatigable exploring it. He admitted that every so often he wished to be dead but also asked me recently “Please continue phoning me, you keep me alive in this way”. He was so in love with life that it seemed that he had to convince himself that such passion must set the pace for death. He wrote in 1973 “Death while it might be explored, can never be fully charted”.

Ed. has been a friend and mentor for a number of years. Despite a big generation gap, we got acquainted and developed a mutual sincere interest.  He was always kind, sweet, polite, and articulate as well as kindly severe for things that didn’t approve. His dedications to me in letters and books are really precious drops of his enormous wisdom that I will never forget.  In his latest book had written on October 2008 “To Maurizio Pompili, my treasured Italian son, with my deep pride in your accomplishments and sweet gratitude for your loyalty. Con Amore, Ed”. When meeting him in Los Angeles last year I found a man packed with memories and reminiscences of any kind. He had sent me a message on the occasion of the Shneidman Award saying “I’m thrilled to pieces at your receiving the award. No one is more deserving and your being the awardee warms the cockles of my hearth in a very special way. The Shneidman Pompili connection foretold in the Suicidological Stars. With my warm embrace. Ed”. I discovered a truly beautiful human being that was grateful for what life had given to him.  His house was full of recollections related to the great love for his family and for his beloved wife as well as with signs of his interest for Melville and Murray. During the conversation you could appreciate the emotions of a sensible man that gets excited by simple things. He always stressed the need to include a mentalistic approach when trying to understand suicide. It is the view of a person that never gave up the mission to ameliorate the psychological drama occurring in suicidal individuals. Suicidology has lost a charismatic figure that changed the view of suicide over the past decades. From the many messages, I got convinced that all the people involved in suicide prevention are somewhat united in paying a tribute to this man.

La U.O.C. di Psichiatria dell’Azienda Ospedaliera Sant’Andrea di Roma, Cattedra di Psichiatria, Facoltà di Medicina e   Psicologia, Sapienza Università di Roma, è allo stato attuale uno dei principali poli al mondo di ricerca scientifica nel campo del suicidio. La consolidata tradizione in questo ambito hanno reso al gruppo alta credibilità e stima nei massimi ambienti scientifici internazionali permettendo collaborazioni con numerose istituzioni universitarie e non, nelle quali hanno sede sviluppi scientifici di grande rilievo. Non meno importante è l’impegno che la suddetta struttura effettua nel campo del suicidio grazie alle interazioni con l’International Association for Suicide Prevention, l’American Association of Suicidology, l’International Academy of Suicide Research e l’Organizzazione Mondiale della Sanità.

Progetti in corso

• START Study. Studio condotto in collaborazione con l’Organizzazione Mondiale della Sanita’. Il Servizio per la Prevenzione del Suicidio è il referente per l’Italia dello START. Lo studio prevede 4 moduli: 1) monitoraggio epidemiologico su suicidio e tentativo di suicidio; 2) randomizzazione di pazienti con tentativi di suicidio in un braccio che prevede un intervento breve di terapia confrontato con un braccio che prevede il “treatment as usual”; 3) studio di autopsia psicologica che prevede un’intervista sistematica a coloro che hanno perso un caro per suicidio; 4) monitoraggio dei tentativi di suicidio ad alta letalita’.

• Follow-up di pazienti ricoverati in SPDC. Studio che recentemente e’ confluito nel Progetto PRIN - prevenzione del rischio di suicidio in pazienti psichiatrici ricoverati e nel periodo successivo alla dimissione. 

• Valutazione ed outcome di pazienti psichiatrici ambulatoriali. Stima della disperazione, temperamento, depressione e rischio di suicidio.

• Rischio di suicidio e iperattività dell'asse ipotalamo -ipofisi-surrene in pazienti affetti da Disturbo Depressivo Maggiore . Lo studio si propone di valutare soggetti con diagnosi di DDM e con storia di TS o ideazione suicidaria e pazienti affetti da DDM con presenza di sintomi psicotici.

• Valutazione dell’autoinganno come costrutto correlato al rischio di suicidio in pazienti psichiatrici ricoverati e in soggetti della popolazione generali.

• Correlazione tra polimorfismi genetici  e rischio  di suicidio, temperamento e disturbi psichiatrici. Studio innovativo che prevede la valutazione di tutti i possibili polimorfismi  che possono essere associati a rischio di suicidio. Lo studio in corso è in collaborazione con il Dipartimento di Biologia Molecolare Avanzata.

• Studio di valutazione del cortisolo salivare in pazienti psichiatrici ricoverati e associazione con rischio di suicidio. Studio  realizzato con la Cattedra di Farmacologia.

• Studio randomizzato controllato in doppio cieco per  la valutazione dell’impatto degli Omega-3 PUFA in pazienti anziani  depressi affetti da patologia cardiovascolare. Realizzato e in collaborazione con San Raffaele, Roma.

• Studi epidemiologici sul suicidio in Italia in collaborazione con l’Istituto Superiore di Sanita’. Valutazione e stima degli indicatori associati al rischio di suicidio nella popolazione italiana. Stima dei tassi e dei trend nel corso di serie storiche. Valutazione e stima dell’omicidio in Italia.

• Progetto intervento inerente la campagna di prevenzione della depressione e del rischio di suicidio promosso dall’Alliance Against Depression. Il SPS è referente per l’Italia del seguente studio. Progetto europeo con il coinvolgimetno di 9 partners, che mira a formare e ad educare operatori della salute e della comunità sulla depressione e il rischio di suicidio.

• Validazione di strumenti psicometrici con indagini sulla popolazione generale e su popolazioni selezionate.

• Valutazione della qualita’ di vita, rischio di suicidio e azione dell’efavirenz in pazienti HIV positivi.

• Valutazione dell’efficacia del trattamento di psicoeducazione in pazienti a rischio di suicidio.

• Valutazione con risonanza magnetica funzionale di pazienti psichiatrici ricoverati con particolare riferimento a confronti  tra soggetti a rischio di suicidio versus soggetti non a rischio.

• Realizzazione di review sistematiche inerenti i  seguenti argomenti: suicidio e ECT, efficacia della stimolazione magnetica transcranica, serotonina piastrinica e rischio di suicidio; effetti indesiderati degli antipsicotici; abuso fisico e sessuale e rischio di      suicidio; assistenza ai survivors; gli incidenti stradali e i suicidi; abuso di sostanze e rischio di suicidio in adolescenza; il disturbo post-traumatico da stress e suicidio nei veterani; rischio di suicidio in pazienti con primo episodio psicotico.

• Intervento di prevenzione del suicidio nelle carceri. Ricerca realizzata presso il carcere di Rebibbia Nuovo Complesso.

• Valutazione della comorbidita’ psichiatrica e dell’outcome dei trattamenti in pazienti affetti da emicrania cronica.

• Valutazione ella comorbidita’ psichiatrica e dell’outcome dei trattamenti in pazienti affetti da glaucoma.

• Valutazione del temperamento e della depressione in  un campione di donne subito dopo il parto. In  collaborazione con la cattedra di Ostetricia e Ginecologia.

• Progetto Stigma presso il territorio di Rieti con intervento in scuole medie superiori.

• Stimolazione magnetica transcranica. Questo studio si propone di dimostrare come la stimolazione transcranica possa indurre una remissione parziale ed alle volte totale della sintomatologia , in pazienti resistenti ad una terapia farmacologica.

• Studio sulla genetica ed epigenetica del comportamento suicidario in pazienti affetti da disturbi dell'umore. Il presente studio si propone di effettuare una valutazione prospettica del comportamento suicidario come fenotipo primario in pazienti con disturbi affettivi. Verrà inoltre effettuato un follow up sugli individui inclusi nello studio con interviste telefoniche annuali per cinque anni al fine di rilevare la presenza del rischio di suicidio.

• Rischio di suicidio e patologie di interesse psichiatrico in pazienti in regime di ricovero ospedaliero. Medicina liaison. Lo studio si propone di valutare il rischio di suicidio in pazienti di interesse internistico e valutare le eventuali correlazioni esistenti tra patologie internistiche e patologie di interesse psichitrico.

• Teen Screen Program. TSP è un programma nazionale di salute mentale per la prevenzione del disagio e del suicido in adolescenza. Questo programma è stato realizzato presso la Columbia University di NewYork, Dipartimento di Psichiatria dell’Infanzia e dell’Adolescenza. Il TSP è un programma di screening volontario che prevede la somministrazione di due questionari di screening, di una debriefing interview di un intervista clinica per i soggetti a rischio di suicidio.

Il Servizio per la Prevenzione del Suicidio offre un numero telefonico diretto per
raccogliere le richieste di aiuto. Il nostro scopo è fornire alla collettività un servizio
di prevenzione del suicidio rivolto a chi sta attraversando un momento di crisi emotiva
o a chi ha perso un caro per suicidio (survivors), attraverso una linea telefonica
sostenuta da un’équipe di medici, psicologi e volontari. Da tutta Italia, quotidianamente,
riceviamo richieste di aiuto che celano una disperazione non facilmente condivisibile,
vissuta spesso con estrema solitudine. Questo servizio vuole offrire, sia
a coloro che sono a rischio di suicidio sia ai survivors, la possibilità di accedere ai
nostri programmi di prevenzione presso i Servizi ambulatoriali della nostra Azienda
oppure ai presidi territoriali di zona.
